¨     Number of people attending the Friday evening meeting

¨     Number of people attending the Saturday dinner (remember no more than 2 guests may attend with you, and childcare is not provided)

¨     Number of people attending Monday evening reception

¨     Number of children who will need childcare during Monday evening reception

¨     Desired college major

¨     Debit card (for the Welcome Weekend/NSO fee)
